Khun Ked have treated my patella/knee football injury 3 times so far and the service have been professional and invaluable in my recovery so far. There is still a way to go, before I can play again, but I can highly recommend the physio services here.
Form Physio and Rehab is a great place at Sukhmvit 22, to have any of your physical injuries checked and improved. The physiotherapist will have a good look at what your problem is and she will improve the situation in any way possible. After that, a private rehab trainer/coach, who in my case was Josh, will strengthen that particular body part where the injury took place, and also focus on the surrounding body parts, like the whole body will be able to withstand on it’s own. I highly recommend anyone with any psychical irritation or injury to go to this center, as it will definitely improve the current situation.
After a year of pain and reoccurring injuries (shoulder, neck, lower back, sciatica), I was recommended to see Maz at Form Physio and Rehab. and after 10 weeks/20x of personal training and adjustments from Maz and the physio, I feel so much better all over. After suffering from a back problem, Maz got me back to being strong. Within 4 months, I went from lying down at home and not being able to move to deadlifting 1.5x my weight. As a Coach, he was persistent in seeing results, positive, and kept me accountable. His life seems to revolve around health and fitness (and good food) and even more so around seeing his clients healthy and strong.
After my assessment with Forms physio, I had been made aware that even I had a muscular imbalance. My left tibialis anterior (shin muscle) was weak following an ankle injury I had suffered several years ago while playing football. This meant that my ankle was at risk to get injured again every time I played. However, by crafting a program specifically for our needs and goals, Josh has helped us regain strength in the muscles that have been weakened over the years. Now, when I step on the football pitch, I feel much more powerful, quicker, agile, stronger, more balanced, but most of all I feel confident again. As a passionate football fan, I feel as if I can now express myself with freedom on the pitch. After a 12 week course, I think it’s fair to say that the both of us are much happier and healthier. However, this doesn’t even come close to what Josh has truly helped me with. In essence, he’s helped me improve my lifestyle. He’s helped me realize the depth of the saying “you are what you eat”. I used to feed off of sugar rushes and unhealthy junk food, but now since I’ve cut the crap out of my diet of stopped feeling like crap. Josh being a certified nutritionist, he’s helped me construct a healthy diet that not only supports muscle growth, but also helps me feel good and vibrant on a daily basis. All in all, he’s helped me become a better version of myself. Fitness has had an extremely positive impact on my life and wellbeing in a number of ways, and I can’t thank Josh and Form enough. Although I’ve made a lot of progress in the last 7 months, I still have a long road ahead of me. My advice to anyone who trains with Josh or any other Form trainer in the future is to attack each workout with more intensity than the last, eat every meal with more discipline than yesterday, sleep with more restfulness than the past nights. Give him a chance. Let him do his job. Take his advice, but also know that he can only do so much. In the end, it’s what you do outside of the gym that will determine how effective your course has been.
I have had only 5 sessions here so far and it has made a huge difference to my knee and confidence already. I can already do many things I couldn’t before with less pain. I will continue to visit Form Physio Rehab and highly recommend it to everyone. They are very professional and the best place I have been to in Thailand. Thank you very much Ked 🙂
I have had It band issues, hip and back pain for years. This got so bad early this year that I had to completely stop running.
I have tried other physios previously that hasn't been able to help me, but then I met Khun Ked. A few months with her and I'm back running and have a lot less back pain in the office. It is getting better by the week and I now feel hope that I'm actually is going to become completely pain free.
I’ve suffered from knee problems due to my reconstructive surgery of a torn ACL and meniscus two summers back. After getting my legs thoroughly checked by the FORM physiotherapist, I decided to undergo personal physical therapy and training sessions. By signing up for personal sessions with Maz as my trainer, I had the opportunity to cater my workouts specifically to me in hopes of regaining equal balance as well as leg and core strength. Maz slowly increased both the intensity and creativity of exercises in each personal session, resulting in immediate improvements in my self-efficacy, posture, and ability to perform simple exercises. Regular physio check ups at FORM throughout my training would reaffirm these results too. Eventually, I returned to the football field and experienced a newfound sharpness, quality of performance, and confidence in my play. In the end, I feel extremely privileged to not only have but also take the opportunity to make major strides in my rehabilitation and overall health goals, especially for someone like me who “lives to eat”. More than anything else, I can take a plethora of lessons from my memorable experiences at FORM in appropriately designing my exercises and shaping my diet now that I have returned abroad. I feel more motivated to continue on the same path that FORM has paved for me, and fully advocate for absolutely anybody who is looking to get stronger, fitter, and healthier. I appreciate all the help from the FORM team, and want to extend an extra special thanks to my new friend Maz for showing an undeniable passion and motivation to serve the needs of his community. When you’re recovering from a major injury, you want to work with a knowledgeable rehab specialist who is equal parts challenging, patient, friendly and professional. That’s been my experience with Josh 100%. I’ve made tremendous progress over the past two months recovering from a broken patella and am positive I’ll reach my goal of being completely recovered sooner rather than later. A big fan of what Form Physio and Rehab is doing overall and feeling blessed that I discovered them when I did!

Ked was very helpful. When I first came after a sports injury, I couldn't put my full weight on my foot. After 4 or 5 weeks I am walking out able to return to rugby!
"When I first started at Form, I was doing personal training and had a basic fitness goal of reducing body fat and increasing muscle mass... However, after a few weeks of training, I began to notice that I was very injury prone despite doing the most basic exercises. For example, my knees would hurt from squatting and my shoulders would hurt from pushups, all despite doing it with correct form. Also, my right hip would often spasm from fatigue. With the help of Maz, Josh and the Physio team I found out that my hip injury from 10 years ago was what caused my knee joints and hip spasms. Furthermore, the muscular imbalance between my chest and back muscles were causing my shoulder pain. Pretty much, I was unable to activate my muscles efficiently, specifically in my glutes, back, shoulder and leg muscles and therefore my joints would take a lot of the load while doing these basic exercises. After I was diagnosed, Maz and his team drew out a perfect rehab plan for me which included a blend of rehab training, physiotherapy and nutrition advice (to reduce inflammation). After 3 months of the right guidance and my most recent appointment with the physiotherapist, I am ready to switch back to personal training and regular fitness. I cannot emphasise how awesome and seamless the physiotherapy and rehab training experience was."
For 2 months I was visiting multiple physios/doctors/”experts” to diagnose/solve my tennis elbow without success. And it wasn’t until a few sessions at Form Phsyio and Rehab that I was able to fully recover. The reasons for this is because 1) The therapy she applied worked, alleviated the strain and more importantly 2) Her instructions on what exercises/movements to do in ones own time to ensure complete recovery. It is the combination of these two points that makes me happy to recommend Form Physio and Rehab to everyone.

Please check out this clinic and fitness on Sukhumvit soi 22. Ask for Josh Madeira. He fixed my Mothers knee pain which was due to weak muscles and Piriformis syndrome and he stopped her shoulder pain and increased her shoulder to normal range of motion that it used to be in about 8-12 weeks time. We tried many different Doctors and specialist before this and they didn't really help. I highly recommend this place and Josh. 5 Stars!
I wanted to lose weight and gain muscle. I tried exercising by myself only to realize that I didn’t know what i was doing. i decided to look for help from experts which made me come across Form Physio Rehab. I had the privilege of learning and working with Josh who taught me different techniques that engaged more than one muscle group creating an effective training program for me. Besides the new techniques, Josh corrected my form, making me realize that when I was exercising by myself, I was at a risk of injuring myself badly. I highly recommend this place, especially for beginners who want to make a change in their life to learn new techniques and the right form. I lost 12 kgs within the span of 6 months and have never felt stronger. Josh fixed my form, taught me new techniques and most importantly fixed my back. Thank you buddy.
